2019 Community Health Improvement Planning (CHIP) In Lake County

The Florida Department of Health in Lake County is inviting the public to help support a culture of health in the community. Opportunities to participate include upcoming community health improvement plan (CHIP) webinars, team events, and focus group meetings.
2019 Focus Group Conference Calls – fourth Friday of every month. Time:
9:00 AM – 10:00 AM (Obesity focus group)
10:00 AM – 11:00 AM (Substance Abuse focus group)
11:00 AM – 12:00 PM (Access To Food & Nutrition focus group)
2:00 PM – 3:00 PM (Access To Primary Care focus group)
3:00 PM – 4:00 PM (Diabetes focus group)
